In the Oncology and Infusion Therapy Clinic, a case manager works on the patient’s behalf, serves as an advocate for patients and families, helps triage and manage symptoms and works closely with the patient and all caregivers to help obtain access to needed hospital and outpatient services. Coordination of Care
The University of Minnesota Physicians Radiation Therapy Center at Fairview Lakes Medical Center and the Fairview Lakes Oncology and Infusion Therapy Clinic work cohesively as a team to provide patients with safe and effective oncology services. Because they are located only steps apart, hand-off communication and physician-to-physician discussion take place on a regular basis. Multidisciplinary care conferences are held regularly to coordinate critical aspects of each patient’s care.
